TUALATIN - Tualatin High School has set a fall sports registration day to be held Wednesday in the commons area at the high school.

Registration times are 8 to 10 a.m. for incoming freshmen and seniors and from 11 a.m. to 1 p.m. for incoming sophomores and juniors.

Fall sports athletes at Tualatin will need to fill out and turn in a Tigard-Tualatin School District athletic clearance form. It will need to be completed and signed by athletes and parents.

All athletes must also have medical insurance coverage before they will be allowed to try out or practice. School insurance forms will be available at the fall sports registration day if you want to purchase it.

The Oregon State Board of Education requires athletes to have a sports physical every two years. Incoming freshman athletes and student athletes new to Tualatin are required to have a physical dated after May 1, 2011.

There is also a $225 pay-to-play fee per sport, plus a $5 refundable athletic lock fee that will need to be paid.

Athletes will not be registered to try out or practice for sports until the required forms are brought to the Tualatin athletic office. A current physical, health insurance and sports fee are required before you are allowed to try out or practice.

Fall sports practices start on Aug. 22.
