   NOTIFICATION OF MEDIA ACCESS TO STUDENTS
    This is notification that the following rules shall apply to media contact with students during school, in transit to and from school activities, at school activities, and at any time on school property: 1) Any media contact with students except for events and activities attended by the general public must be pre-arranged with the school principal. Access may be denied if the principal thinks it could result in undue disruption of school activities. 2) The media may interview and photograph students involved in instructional programs and extra-curricular school activities. 3) Information obtained by media representatives directly from students does not require parental approval prior to publication. Parents who do not want their student interviewed or photographed may direct their student accordingly and may submit a written request to the principal. 4) District employees may only release student information to the media in accordance with the District's student records policy - JO.

   NOTIFICATION OF RELEASE OF DIRECTORY INFORMATION
   This is notification that Directory Information about your student may be released. This information is not generally considered harmful or an invasion of privacy. The following categories are designated as Directory Information: Student's name; date and place of birth; participation in officially recognized sports and activities; weight and height of athletic team members; dates of attendance; degrees and awards received; most recent previous school or program attended.
   Directory information also includes a student's photograph. Parents may request that directory information not be released by writing a dated letter stating such to the student's school principal.

   NOTICE OF DRUG FREE WORKPLACE
   AND DRUG FREE SCHOOLS
   Jefferson County School District 509-J has established Board Policy GBEC - Drug Free Workplace and Drug Free Schools. The Board believes in the total development of students and employees. Abuse, possession, use, sale or furnishing alcohol and illegal drugs, or other controlled substances at the workplace or in the schools is potentially disruptive and poses a threat to the educational process. As a mandatory condition of employment in Jefferson County School District 509-J, employees agree to abide by the terms of this policy and to notify the Superintendent of any criminal drug statute conviction for a violation occurring in the workplace no later than five (5) days after such conviction. Student violations will be handled as disciplinary matters under Board Policy JFC - Student Conduct, Board Policy JG - Student Discipline, Board Policy JFCI - Substance Abuse.

   In accordance with Oregon Revised Statutes 336.645, Jefferson County School District 509-J hereby informs all parents, students, or guardians of the alternative education law. The appropriate administration of any district school will provide written notification to parents, students, or guardians about the availability of alternative programs as outlined in Board Policy IGBHB-AR - Rules and Procedures for Parents and Students on Alternative Education Programs.    NOTIFICATION OF PARENTAL RIGHTS
   This is the annual notification to parents/guardians of students in the Jefferson County School District 509-J, of their rights in regards to student education records. Parents/guardians have the right to:
   a) Inspect and review the student's education record;
   b) Request the amendment of the student's education record to ensure they are not inaccurate, misleading, or otherwise in violation of the student's privacy or other rights;
   c) Request no disclosure of personally identifiable information contained in the education records of the student;
   d) File a complaint with the U.S. Department of Education concerning alleged failures by the District to comply with the requirements of the Family Educational Rights and Privacy Act;
   e) Obtain a copy of the District's adopted Student Education Records Policy JO at the School District Office at 445 SE Buff Street, Madras; and
   f) Receive translation assistance if requested.

   NOTIFICATION OF TRANSFER OF STUDENT EDUCATION RECORDS
   This is notification to parents/guardians that the Jefferson County School District 509-J within 10 days of a legitimate request, will transfer to the requesting school, educational institution, or educational agency those records identified as student education records maintained by the School District and the E.S.D. Prior to the withdrawal of a student from the School District, parents/guardians have the right to inspect their child's records and challenge information contained in the record they believe is inaccurate, misleading or in violation of the privacy or other rights of the student.
   Requests to review a student's records and/or the District records policy are to be made by the parent/guardian to the student's building principal. If requested, translators will be made available for non-English speaking parents.

   EARLY KINDERGARTEN ENTRY (Revised)
   Resident children who do not meet minimum age requirements may be admitted if they qualify under the definition of educationally talented and gifted in ORS 343.395. Parents of a four-year old child born between September 1 and September 30 may apply for early entry into school following the child's successful completion of assessment. Students born after September 30 are not eligible for early entrance consideration. Between April 15 and August 15, applications for early entry shall be made to the school building principal containing a letter of application from the parent including the child's name, address, birth date, and reason for early entry request; a birth certificate verifying minimum age of the child to be 4 years 11 months by September 1 in the year of entry; and the child's immunization record.
